Application Form for Candidates to the Short Film Academy Register

I consent to the processing of my personal data (surname, first name, patronymic, date of birth, citizenship, contact details, information on professional activity and academy membership, photograph) for the purposes of reviewing my candidacy and including me in the "Academy Persons" register, as well as for maintaining the register. I consent to the dissemination of my personal data to an unlimited number of persons by publication in the "Academy Persons" register on the website www.eternitypictures.ru of the following data: surname, first name, patronymic; photograph; information on profession and qualification category; biography; information on membership in film academies. Silence or inaction does not constitute consent; I hereby confirm that this consent is given by me voluntarily and knowingly.

I consent to the publication and use of my photograph on the website www.eternitypictures.ru and in Academy materials (including advertising and promotional materials of the Award) for the purpose of presenting me as an Academy Person. This consent is valid for 5 years from the date of signing or until withdrawn, applies worldwide, without limitation as to the methods of use. I may withdraw my consent in writing.

I consent to the use of my name (surname, first name, pseudonym) in the "Academy Persons" register and in Academy materials in ways that do not mislead third parties as to the identity of citizens and do not affect my honour, dignity or business reputation, for the duration of my inclusion in the register.

I confirm that all information provided in this application is accurate, that documents on academy membership are submitted in original form or certified, and that I am entitled to use the stated titles and memberships.

I confirm that my participation in the register and in the expert activities of the Short Film Academy does not contradict the charters, internal rules or codes of ethics of the film academies and organisations of which I am a member, and does not violate my obligations to them.

I have read and agree to the Regulations of the "Academy Persons" register, the Privacy Policy and the rules of participation of Persons in the expert activities of the Academy.

I confirm that inclusion in the "Academy Persons" register is carried out on a voluntary and gratuitous basis at my own initiative, without any payments in my favour, and does not create membership in any association.

I confirm that I am the person who has completed this form, that I have read the User Agreement and the Agreement on the Use of a Simple Electronic Signature (SES), and that I am signing this application and all consents and terms set out in the form with a simple electronic signature. The identity of the signatory is determined by the combination of data specified in the form and recorded in the Organiser's CRM system (surname, first name, patronymic; email address; token; IP address; date and time of submission; device type), in accordance with Section 3 of the Agreement on the Use of a Simple Electronic Signature. To sign, a unique one-time code (token) has been generated for me; I enter it in the relevant field and click the "Submit Application" button. The moment of signing is deemed to be the moment the "Submit Application" button is clicked and the data are recorded in the Organiser's CRM system. An electronic document signed with such a simple electronic signature is recognised by me as equivalent to a paper document with a handwritten signature.
